1. Acceptance of Terms

By booking an appointment with SOUL BALANCE Diabetes & Endocrine Centre of Excellence (“Soul Balance”), you agree to these Terms & Conditions. If you do not agree, you should not use our appointment booking services.

2. Eligibility

  • Patients must be 18 years or older to book an appointment.

  • Patients under 18 may only book appointments through a parent or legal guardian.

3. Appointment Booking

  • Bookings can be made via our website form, WhatsApp, or other official communication channels.

  • Bookings require submission of your Name, Email, WhatsApp Number, Preferred Date, and Time Slot (Afternoon/Evening).

  • Appointment confirmations will be shared via email and/or WhatsApp notifications.

4. Payment Policy

  • All consultation fees are to be paid directly at the clinic at the time of your visit.

  • No online payment is collected through the booking form at present.

5. Cancellation & Rescheduling

  • Patients may cancel an appointment by notifying the clinic in advance via WhatsApp, email, or phone.

  • The clinic reserves the right to reschedule or cancel appointments in case of unforeseen circumstances or doctor unavailability.

  • Updated timings or cancellations will be communicated through WhatsApp and/or email notifications.

6. Follow-Up Reminders

  • Soul Balance may send you reminders for follow-up consultations (e.g., next month or next week) via WhatsApp or email.

  • By booking an appointment, you consent to receive such communications.

7. Communication & Notifications

  • By providing your contact details, you authorize Soul Balance to send you appointment confirmations, reminders, and service-related notifications via email, WhatsApp, or SMS.

  • Messages may be delivered through secure third-party communication providers.

  • You may opt out of non-essential communications by contacting us (see Section 11).

8. No Emergency Services

  • Soul Balance does not provide emergency medical services.

  • In case of a medical emergency, please contact your nearest hospital or dial local emergency numbers.

9. Limitation of Liability

  • Soul Balance and (Placeholder) shall not be held liable for:

  • Technical issues, delays, or failures in appointment confirmations or notifications.

  • Missed appointments due to incorrect information provided by the patient.

  • Any indirect, incidental, or consequential damages arising from the use of our booking system.

10. Data Protection & Privacy

We collect only the information necessary for booking and communication: Name, Email, WhatsApp Number, Appointment Date & Time Slot.

No medical records are collected or stored.

Your data is stored securely and used solely for:

 

Appointment scheduling, confirmations, cancellations, and reminders.

Follow-up consultation reminders.

Communication regarding clinic services.

 

Data may be processed through secure third-party service providers to facilitate notifications.
